Annual Valuation Services

 


 

In order to complete the annual valuation we require the following materials from each client:

 

1.      Update enclosed anniversary census list indicating total compensation (W-2 wages), hours worked during the plan year, date of birth, date of hire and date of termination for terminated employees;

2.      A list of contributions paid during the plan year, including the date(s) and amount(s) of each contribution payment(s);

3.      A list of contributions paid or to be paid after the plan year end to be applied to the previous plan year;

4.      A list of trust disbursements during the last plan year to include: insurance premiums paid listed by amount, participant name and to which insurance company indicated, expenses paid, benefit payments including the name of payee, date paid and amount paid;

5.      A copy of year end statements from each regulated financial institution holding or issuing qualifying plan assets and a list of all non-qualifying plan assets with their year end market value (i.e. real estate, mortgages, etc.);

6.      Name of Insurer supplying the Trustees’ Fiduciary Bond and the amount of coverage.
